“Long-lasting” gets thrown around a lot in medspa marketing, but treatments vary enormously in how long their results actually hold up, and the honest answer is always “it depends on you.” Still, some categories of treatment are built for longevity, and others aren’t. Here’s a realistic ranking, shortest to longest, based on how these treatments actually work.
Botox and other neuromodulators temporarily relax the muscles that cause expression lines. Because the effect depends on nerve signals gradually returning to normal, results typically last up to four months before a touch-up is needed. Newer options like Daxxify are formulated to potentially stretch that window further, though individual results still vary. Neuromodulators aren’t the longest-lasting category, but they remain one of the most predictable and reversible, which is part of why they’re such a popular starting point.
Fillers like Juvederm, Restylane, and the RHA Collection add volume directly and are gradually broken down by the body over time. Depending on the specific product, the treatment area, and your individual metabolism, results generally last six months to about a year and a half. Areas with more movement, like the lips, tend to metabolize filler faster than lower-movement areas like the cheeks.
Radiesse works differently than a traditional HA filler. Its calcium hydroxylapatite microspheres add immediate volume, then stimulate your own collagen production over the following months. That combination of instant correction and collagen stimulation gives Radiesse results commonly cited at around two years, longer than most standard HA fillers, with some patients reporting results lasting even beyond that.
Sculptra takes the opposite approach from an instant filler: rather than adding volume directly, it uses poly-L-lactic acid to gradually stimulate your body’s own collagen production over a series of sessions. Because the results are your own rebuilt collagen rather than an injected substance waiting to be metabolized, Sculptra results are commonly reported to last up to two years or more, making it one of the longer-lasting options in the injectable category, at the cost of a slower, multi-session timeline to get there.
PRF uses your own blood plasma to stimulate collagen and improve skin quality and tone over time. It’s not a volumizer in the same sense as a filler, so “how long it lasts” is a slightly different question, results build gradually and tend to improve overall skin quality in a way that compounds with consistent treatment and good skincare, rather than fading on a fixed timeline the way a filler does.
Devices like Morpheus8 (radiofrequency microneedling) and laser treatments such as Halo or BBL work by triggering your skin’s own remodeling and collagen response. A single session can produce improvements that last a year or more, and because these treatments improve the skin’s underlying structure and texture rather than just adding volume, consistent maintenance sessions can extend and compound results over multiple years. These aren’t typically “one and done,” but the cumulative, long-term skin quality improvement is where they really outperform any single injectable.
Advanced laser treatments such as radiofrequency microneedling with Morpheus8 or resurfacing with Halo and BBL Hero work by triggering your skin’s own natural collagen response.
There isn’t one single “longest-lasting” treatment that’s right for everyone. Neuromodulators offer predictable, reversible, shorter-term results ideal for testing the waters. Radiesse and Sculptra offer real longevity through collagen stimulation, at the cost of a longer commitment upfront. Energy-based treatments and PRF play a longer game entirely, improving the skin itself rather than just correcting a specific line or area. The right answer depends on your goals, your timeline, and how much maintenance you actually want to keep up with.
